The Health Center of Southeast Texas opened its doors in 2006 to provide affordable healthcare for our community.

Our main goal is to address the cultural and financial barrier to health care access in Liberty county and adjacent areas by providing a sliding scale fee and community based primary care service.

In September 2007, we received Federally Qualified Health Center status which allowed us to grow and expand our services. We now have two locations, four providers, and many programs that allow us to bring affordable health care to the communities we serve. HCOSET is an independent, non-profit 501(c) 3 clinic governed by a board of directors. The clinic is open to all.

We accept Medicaid, Medicare and almost all the major insurances. We have sliding scale fee based on the household income for the uninsured.

The Health Center of Southeast Texas receives income from three sources: State and Federal governmental grants, Patient fees and Private contributions.

PCMH Statement

The Health Center of Southeast Texas is working on becoming a Patient-Centered Medical Home to enhance the practice and delivery of primary care. The Patient-Centered Medical Home (PCMH) places the patient and primary care practice at the center of care and creates a healthcare setting that is a collaborative partnership between the patient and their personal physician.
